Print off Registration Forms:              Membership Application            Medical History/Release            Player Agreement            Parent/Guardian Agreement            Hit-A-Thon Forms Complete and sign all forms, include copy of birth certificate (and photo ID such as driver's license for 16/U and 18/U), check for payment of dues if not fully participating in fundraising, and mail it to CGFPA, P.O.
